  1. Please include proof of your nonprofit (501c3) status
     
  2. Please provide a brief description of the organizing group.
     
  3. Please briefly answer all of the questions below. See the RFI Overview for additional details about the types of learning communities we are seeking. You can either be part of an existing learning community or want to create one.

    A. WHY: Why did you or why do you want to create a learning community that includes Mind in the Making: The Seven Essential Life Skills Every Child Needs?


    B. WHO: Who are/will be the organizers/leaders? Who are/will be the participants? How are you bringing together groups that typically may not work together across the birth through eight continuum (for example, principals and early childhood directors / parents and teachers, etc.)? How are you reaching out to underserved communities? 

    C. WHAT: What are/will be your goals?  What are/will be the strategies you use to meet these goals?  What are you doing/will you do to bridge the birth through eight continuum?


    D. HOW: How does/will your learning community work? Please describe so that we can get a picture of what your learning community is like.
     

  4. What lessons have you learned in creating a learning community? What works, what doesn’t? Please be as specific as possible since we want to share your lessons learned.

     
  5. How much are you requesting in the mini-grant? Please detail your expected expenditures. Note: we will provide UP TO $5000 to community, state, or national groups (groups do NOT need to request the full amount. The smaller amount of the mini-grants, the more learning communities we can fund).  How do you plan to use these funds if you are selected? Please provide a summary budget. Note that only expenses directly attributable to the grant activities are allowed; therefore, a general overhead percentage or indirect costs are not allowed.
